| LAMal (assurance suisse) | CMU (assurance maladie française) | |
|---|---|---|
|---|---|---|
| 30 000 € | ≈ 120 € | ≈ 214 € (200 CHF) |
| 40 000 € | ≈ 187 € | ≈ 214 € |
| 50 000 € | ≈ 253 € | ≈ 214 € |
| 60 000 € | ≈ 320 € | ≈ 214 € |
| 80 000 € | ≈ 453 € | ≈ 214 € |
| 100 000 € | ≈ 587 € | ≈ 214 € |

CMU or LAMal in 2026: which health insurance should a cross-border worker choose?
When you start working in Switzerland while living in France, you must choose between Swiss health insurance (LAMal) and French health insurance, commonly called the cross-border CMU. The choice is time-limited and in principle final. Here is how each system works, what they cost in 2026 and how to decide.
The right of option in brief
- You have 3 months from the start of your job in Switzerland (or your move to France) to choose, according to the canton of Geneva.
- The choice is final. There are only rare exceptions, such as cancelling an automatic affiliation under certain conditions.
- If you do not choose within 3 months, you are automatically insured with a Swiss health insurer (LAMal).
- To choose French insurance, you fill in the form "Choice of health insurance system", endorsed by the CPAM, and send it back to the competent cantonal authority within that 3-month period.
How each system works
| LAMal (Swiss insurance) | CMU (French health insurance) | |
|---|---|---|
| What you pay | A fixed monthly premium per person, depending on the insurer and age, not on your salary | 8% of your income from the last known year (in practice N-2), calculated under the reference taxable income rules, after a deduction of 25% of the social security ceiling, paid to Urssaf |
| Your family | Each family member without gainful activity must be insured in Switzerland: spouse and children each pay a premium | A spouse without activity and children are covered as dependants: the calculation formula has no family component |
| Where you can get care | In Switzerland and in your country of residence | In France, at the same reimbursement rates as other insured people; you can also see a doctor in Switzerland without prior authorisation |
What it costs: the 2026 figures
For the CMU, the decree (article D380-2 of the social security code) sets the formula: 8% × (income − 25% of the social security ceiling). Income means that of the last known year, i.e. N-2 in practice, defined under the reference taxable income rules and calculated individually. The Haute-Savoie prefecture gives an example: 2022 income of €57,000 leads to a 2024 contribution of €3,737, i.e. about €311 per month. For LAMal, the cheapest cross-border rate in 2026 is Helsana's: CHF 200 per month for an adult aged 26 and over, and CHF 49.40 per child.
| Annual income used (year N-2) | CMU: monthly contribution | LAMal Helsana: single adult |
|---|---|---|
| €30,000 | ≈ €120 | ≈ €214 (CHF 200) |
| €40,000 | ≈ €187 | ≈ €214 |
| €50,000 | ≈ €253 | ≈ €214 |
| €60,000 | ≈ €320 | ≈ €214 |
| €80,000 | ≈ €453 | ≈ €214 |
| €100,000 | ≈ €587 | ≈ €214 |
Calculated with a deduction of about €12,000 and CHF 1 ≈ €1.07. For an employee, the income used is close to the net taxable salary on the tax notice. Other LAMal insurers are more expensive than Helsana. Rounded amounts, indicative only.
For a single person, the CMU is cheaper up to an income used of about €44,000 a year; above that, LAMal with Helsana becomes more advantageous. With a more expensive LAMal insurer, this threshold rises.
For a family, the calculation changes
Take a married couple with two children, only one of whom works in Switzerland, with an income used of €60,000 a year. With the CMU, the contribution is about €320 per month for the whole family. With LAMal at Helsana, you pay your own premium (CHF 200), your non-working spouse's (CHF 215) and the two children's (2 × CHF 49.40), about CHF 514 in total, close to €550 per month. In this example, the CMU costs about €230 less each month.
How to choose
- Your income: on a modest income the CMU is generally cheaper; on a high income, a fixed LAMal premium becomes advantageous.
- Your family: a non-working spouse and children often tip the balance towards the CMU, which adds no contribution for them.
- How costs evolve: LAMal premiums rise every year (Helsana announced CHF 209.30 per adult for 2027, up 4.7%), while the CMU follows your income.
- Supplementary health cover is optional in both cases and is never tax-deductible.
What is tax-deductible
According to the French tax administration, the LAMal contribution is deductible from your Swiss salary, before conversion into euros. The contribution paid to Urssaf for the CMU is deductible from your overall income, on line 6DD of return 2042-C. For the rest of your return, read Cross-border tax 2026: where are you taxed?
The steps to remember
- Within 3 months: choose your system and, if you opt for the CMU, send back the choice form.
- With the CMU: every year, from September, declare on your urssaf.fr account your income from year N-2, based on your latest tax notice. The declaration is individual: if your spouse is also a cross-border worker insured in France, you each file one. In 2026, the deadline is 14 October for the 2027 contribution (2025 income). Without a declaration, the contribution is calculated on a flat-rate basis equal to five times the annual social security ceiling, i.e. €240,300 for 2026.
- With LAMal: take out cover with a Swiss insurer, and also insure your non-working family members.
Run the numbers for your own situation: Frontier is free and most of it works without creating an account.
